Please reach me at denise@woodsnutrition.com if you cannot find an answer to your question.
I currently offer online virtual visits.
The first session will include a full nutritional assessment such as dietary recall, medical history, anthropometrics, any obstacles, and what your goals are.
Child nutrition counseling will involve a full nutrition assessment as well. I will need to see your child briefly, but it is up to parent discretion if the child will be present or not through out the full visit.
All initial appointments are set up online. You will receive necessary paperwork to fill out prior to your appointment.
In the event that you will be unable to keep an appointment, please provide notice, written or verbal, at least 24 hours in advance. Missed appointments or cancellations without 24-hour notice will be billed for the full session fee.
I am not contracted with any insurance companies at this time. However, if you would like to use your insurance, I will provide you with a Superbill which is a receipt with insurance billing codes, so you can receive reimbursement from your insurance company. I am considered an out-of network provider. If you have further questions, contact your insurance provider to see if my services would be eligible for reimbursement.
